According to the 3+1 system applied in our unit, for participation in the Workplace Training and Workplace Practice courses in the 4th Term, our students need to form a contract with their workplaces and submit or send the relevant documents to our unit by hand or by mail no later than Monday, MARCH 1, 2021.

The process proceeds with the following steps:

1- The student and the workplace official sign and have the acceptance form signed. (attached. It is prepared in 4 copies. Signatures and stamps must be legible.)

2- The documents are delivered to the unit by hand or by cargo.

3- The student's information is reviewed. The Head of the Department accepts or rejects it according to the suitability.

4- The documents are sent to the company.

1- Acceptance forms (4 copies) - Outputs should be prepared as double-sided documents.

2- Copy of identity card

3- The company's full address

Points to be considered

1- The workplace signature and stamp must be legible.

2- The student is responsible for delays in cargo or address differences. Therefore, your shipments should be made very early. The delivery must be completed by MARCH 1, 2021 at the latest. If the document is not approved by the unit, the process cannot be repeated.

3- To accept the workplace, the advisor faculty member or the head of the department must be contacted first.

4- The student, workplace, and faculty member are responsible for any health problems, reports, and leave situations that the student may experience. Therefore, the faculty member must be informed in any situation.

5- The student will need to have an occupational health and safety certificate. This training program will be opened by our university. You should stay in contact with the responsible faculty member.
